Notice of Privacy Practices

THIS NOTICE DESCRIBES HOW MEDICAL INFORMATION ABOUT YOU MAY BE USED AND DISCLOSED AND HOW YOU CAN GET ACCESS TO THIS INFORMATION. PLEASE REVIEW IT CAREFULLY.
Your health information is private. The law requires Bloomline Counseling to protect it, to explain how it is used, and to follow what this notice says.
When your information is used without asking you first
- To treat you — for example, talking with your child’s doctor about care.
- To get paid — giving your insurance what it needs to cover sessions.
- To run the practice — record keeping and professional consultation.
When the law requires information to be shared
- Suspected abuse or neglect of a child, an older adult, or a person with a disability. In Texas, reporting this is required.
- A serious, immediate risk of harm to you or someone else.
- A court order or subpoena.
- An investigation by a licensing board.
- Other narrow situations the law specifically requires.
When your written permission is always needed
- Sharing psychotherapy notes.
- Any marketing use.
- Anything else not listed in this notice.
Your information is never sold. You can cancel your permission in writing at any time.
If the client is a child
A parent or guardian generally has the right to see their child’s records. Therapy with teenagers works better with some privacy, so what will and will not be shared is discussed openly with both of you at the start. Safety concerns are always shared.
Your rights
- See and get a copy of your record.
- Ask for a correction if something is wrong.
- Ask for a list of who your information was shared with.
- Ask that certain information not be shared. If you pay in full yourself, you can require that it not be sent to your insurance.
- Ask to be contacted a certain way or at a certain place.
- Get a paper copy of this notice.
- Be told if your information is ever breached.

Records about substance use
Records protected under federal rule 42 CFR Part 2 get extra protection and cannot be shared again without your written consent, except as that rule allows.
Our responsibilities
Bloomline Counseling must keep your information private, give you this notice, and follow it. If this notice changes, the new version applies to information already held and is posted on this website.
If you think your privacy was violated
Tell Bloomline Counseling directly, or file a complaint with the U.S. Department of Health and Human Services. Nothing bad will happen to you for complaining.
