Your Right to a Good Faith Estimate

Under the federal No Surprises Act, health care providers must give people who are uninsured or who are not using insurance an estimate of the expected charges for medical items and services before those services are provided.

What this means for you

• You have the right to receive a Good Faith Estimate of the total expected cost of any non-emergency items or services. This includes related costs such as assessments and any other services provided as part of your care.
• You can ask Bloomline Counseling, or any other provider you choose, for a Good Faith Estimate before you schedule an item or service.
• If you schedule an item or service at least 3 business days in advance, you will receive a Good Faith Estimate in writing within 1 business day after scheduling. If you schedule at least 10 business days in advance, you will receive it within 3 business days.
• If you receive a bill that is at least $400 more than your Good Faith Estimate, you can dispute the bill.
• Make sure to save a copy or picture of your Good Faith Estimate.

Current self-pay rate

The standard fee for a counseling session at Bloomline Counseling is $155 per session when you are not using insurance. Your written Good Faith Estimate will reflect the services actually planned for you.

If you are using insurance through Headway, you are not covered by this notice in the same way — Headway verifies your benefits and shows your expected cost per session before you schedule.

How to request one

Ask at any time, by phone at (210) 239-8325 or by email at hello@bloomlinecounseling.com. You may request a Good Faith Estimate before scheduling, and you will automatically receive one when you schedule.
